Make An Appointment


Welcome! If you are new to the center, please follow these five steps to better assist you. We look forward to seeing you soon.

1. To schedule an appointment,

please call the center: 310.828.8258

Please let us know:

•Which practitioner you were referred to or would like to see, or which medical modality you would like to use?

•What days and times work best for you?
•What is the reason for your visit?

•How you came to find us?

2. 2.Once you have scheduled your appointment, depending on practitioner or modality, our staff will email or fax you the appropriate intake forms to complete and bring with you to your appointment. To expedite the charting process, either fax to 310.828.5258, email to info@lotusew.com, or drop off the completed forms before your scheduled visit. If you have any questions, our front desk staff is available to assist you.
   

3. Additionally, bring or email all supporting medical records that relate to your health concern(s).

4. 4.Kindly allow enough time for traffic and parking so that you can arrive relaxed and on time for your scheduled appointment.
   
   

5. 5. Cancellation Policy:
   
   *Please understand your appointment time is reserved for you.  We recognize there may be occasions when you need to cancel or reschedule an appointment.  If you need to make any changes to your appointment for any reason, we require a minimum of 24-hour advanced notice to avoid cancellation/rescheduling fees in the amount of your scheduled visit.  Thank you for respecting this policy.*
   
   

     

  A note on parking:

We have outdoor patient parking in the rear of the building (off 21st street, just south of Wilshire Blvd). Do not park in any of the tandem spots along the back/east wall  of the lot. Some read “Tae Kwon Do”. These are reserved spaces and they will, unfortunately, tow. If the lot is full, there is metered parking in the area or non-metered two-hour parking north of Wilshire. Please be sure to read any and all city regulation signs for street cleaning times, “permit parking only”, or any other limitations.
